Participate in Future Health Today

We are inviting forward thinking general practices to join Future Health Today.

Benefits of involvement:
– streamlined identification of patients at risk of chronic disease
– free access to tools to optimise patient recall and care pathways
– streamlined access to guidelines and patient resources
– free training, support and resources from the University of Melbourne
– $600 practice payment as compensation for the short time taken to install the software
– $200 payment for the Practice Champion
– free access to, and licensing of, FHT and GRHANITE® software
– access to training and a quality improvement platform associated with 40 RACGP points
– practice nurses and practice managers have access to certificates towards CPD requirements with APNA and APMA
– staff who participate in feedback, QI or education activities will be eligible to receive CPD points
– potential economic benefits via identification of patients who may be eligible for GP Management Plans and Health Assessments

Your practice can be involved if it:
– has at least one GP and at least one general practice nurse or practice manager (if employed) willing to participate in feedback and education activities
– has a staff member willing to be the Practice Champion and liaise with the researchers
– uses Best Practice or Medical Director (not cloud based)
– uses Windows 10 and can identify a single computer for GRHANITE® and FHT (i5/i7 processor and 16GB RAM)
– has Chrome or Edge installed on all computers
