Quality improvement

Future Health Today uses evidence-based algorithms to identify patients with, or at risk of, chronic health conditions who may benefit from review and optimisation of management. The system provides opportunities to review recommendations for patients individually at the point of care, through active recall of groups of patients, or as part of a broader quality improvement activity.  The quality improvement component of FHT is designed to reduce administrative time taken to plan and record quality improvement activities.

Using the FHT QI Activity Webform, practices can select RACGP accredited QI activities, or create their own QI activity. To claim CPD points on an accredited activity, practices can submit completed activities and associated reports to the University of Melbourne for RACGP points. Practice nurses and practice managers will need to provide a summary of the hours spent on the activities for their CPD requirements. QI activities created by the practices can be directly uploaded by participating GPs onto CPD portals.
